Emergency
ACLS Medications
Epinephrine 1 mg IV/IO every 3-5 minutes
Use: Cardiac arrest (all rhythms) β improves coronary and cerebral perfusion.
Amiodarone 300 mg IV/IO (then 150 mg if needed)
Use: Refractory ventricular fibrillation / pulseless ventricular tachycardia (VF/pVT).
Alternative: Lidocaine 1β1.5 mg/kg IV/IO.
Lidocaine 1β1.5 mg/kg IV/IO (then 0.5β0.75 mg/kg)
Magnesium Sulfate 1β2 g IV/IO
Atropine (no longer routinely recommended for asystole/PEA)
Vasopressin (no longer recommended)

Etomidate 0.3 mg/kg IV
Onset: 30-60 seconds
Duration: 3-8 minutes
Best for: Shock, trauma, unstable patients.
Notes: Single dose only due to adrenal suppression.
Ketamine 1.5-2 mg/kg IV
Onset: 45-60 seconds
Duration: 10-20 minutes
Best for: Hypotension, asthma, bronchospasm.
Notes: Maintains blood pressure and airway reflexes.
Propofol 1-2 mg/kg IV
Onset: 30-60 seconds
Duration: 5-10 minutes
Best for: Hemodynamically stable patients.
Notes: Can cause hypotension β reduce dose if unstable.
Paralytics
Succinylcholine 1.5-2 mg/kg IV
Onset: 45-60 seconds
Duration: 6-10 minutes
Contraindications: Hyperkalemia, burns >24h, crush injury, malignant hyperthermia, neuromuscular disease.
Rocuronium 1-1.2 mg/kg IV
Onset: 60-90 seconds
Duration: 30-60 minutes
Notes: Reversible with sugammadex if available.
